New Process for Student Travel Forms

Risk Management has transitioned the student travel paperwork process to DocuSign to streamline submissions and approvals.

The following forms are now completed and routed electronically through DocuSign. See the Risk Management Forms and Waivers webpage:

  • Student Excursion Authorization Request Form.
  • Medical Information and Emergency Contact Form.
  • General Purpose Waiver, Agreement and Release of Liability Form.

As part of this process, one designated person must be identified by the requesting department when completing the forms. This individual will automatically receive a copy of the completed Medical Information and Emergency Contact Form and the General-Purpose Waiver, Agreement and Release of Liability Form once they are signed to take on the excursion.

Going forward, please use DocuSign for all student travel documentation. This change is intended to improve efficiency, tracking and record retention.

Risk Management is also working to translate these documents into additional languages. We will provide an update once that process is complete.
